Whether it\u2019s an incident (such as an injury, illness, property or equipment damage), near-miss, psychological or workplace violence incident, unsafe work condition, or health and safety concerns, a workplace investigation helps us understand what happened, why it happened, and what we can do to prevent it from happening again.&nbsp;&nbsp;<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Why do workplace investigations matter?<\/strong>&nbsp;<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Workplace investigations help to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Identify root causes and uncover workplace hazards<\/li>\n\n\n\n<li>Prevent similar incidents from happening again<\/li>\n\n\n\n<li>Promote a healthy and safe work environment<\/li>\n\n\n\n<li>Meet regulatory requirements<\/li>\n\n\n\n<li>Build trust with workers through accountability and transparency<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\"><\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>What is a workplace investigation?&nbsp;<\/strong>&nbsp;<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A workplace investigation is a:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Fact-finding process<\/strong> to uncover the cause of a particular incident<\/li>\n\n\n\n<li><strong>Critical, objective, and systematic examination<\/strong> of all aspects of an incident<\/li>\n\n\n\n<li><strong>Tool<\/strong> for identifying workplace acts, conditions, or procedures that need improvement<\/li>\n\n\n\n<li><strong>Key component<\/strong> of the incident reporting process<\/li>\n\n\n\n<li><strong>Preventive measure<\/strong> to help avoid similar events in the future<\/li>\n\n\n\n<li><strong>Reactive element<\/strong> of an overall injury prevention program<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">What do we investigate?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Workplace investigations may be conducted for:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Incidents, near misses, injuries, and time-loss claims<\/li>\n\n\n\n<li>Equipment or machinery failures<\/li>\n\n\n\n<li>Structural or property damage<\/li>\n\n\n\n<li>Releases of hazardous substances<\/li>\n\n\n\n<li>Health and safety concerns raised by workers<\/li>\n\n\n\n<li>Concerns identified by regulatory bodies<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading\">Who is involved in the investigation?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Investigations must involve people who are knowledgeable about the work being performed.
