A person who could transfer with minimal<br>assistance in the morning may not be able to do so in the afternoon. To keep yourself and the<br>person in care safe, it\u2019s important to assess a person\u2019s abilities before and during a transfer.<\/p>\n\n\n\n<h5 class=\"wp-block-heading\">Who does it?<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">The care giver does the point of care assessment. If you\u2019re moving a person, do a point of care assessment first.<\/p>\n\n\n\n<h5 class=\"wp-block-heading\">How do I do it?<\/h5>\n\n\n\n<p class=\"wp-block-paragraph\">A point of care risk assessment has four areas that you cover, with questions in each area.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>A point of care risk assessment for transfers is a quick mini-appraisal you, the health care worker, do to make sure a person\u2019s abilities still match what\u2019s in their care plan.
